Parents of young participants participating in general assessment 1A (“Baseline”) received four questionnaires:

• 1A Birth Questionnaire
• 1A Behavior Questionnaire
• 1A Food Questionnaire

The 1A Food Questionnaire was handed out to the parents during the child's 1A Visit 1 and contained the following sections and subsections:

• Personal data

• Children's diet (FFQ-TNO) (8-12 years)
• Children's diet (general)
